Help strike out ALS in Illinois when you file your state income tax form. Just check LOU GEHRIG'S RESEARCH - LINE 28, LETTER I - on the IL-1040 income tax form and indicate the amount you wish to contribute. If you use a professional to prepare your taxes, please let them know you want to make a contribution to LOU GEHRIG'S RESEARCH

This is a true grass-roots effort and we need your help in spreading the word to Illinois residents. Please forward this email to all of your Illinois relatives, friends, neighbors, co-workers and acquaintances and ask them to make at least a $10 contribution. The Lou Gehrig's Disease Research Fund must receive a minimum of $100,000 each year to remain on the Illinois income tax form, so everyone's participation is crucial!

Funds raised through the tax check-off program will support ALS research programs in Illinois. So this year, step up to the plate at tax time and check LOU GEHRIG'S RESEARCH on your tax form.
