Care for Life
Care for Life provides ALS patients with grants for free limited in-home respite care. It is made possible through the efforts of fellow PALS and our supporters. Much of the money is raised during the annual Ride for Life. This program will greatly assist families by easing the strain, which many face when dealing with ALS because of the lack of respite care from health insurance providers.
Care for Life provides grants of $1500 each to help families pay for respite care which is provided by a local healthcare agency or professional. Applications are only accepted online and a limited number of grants are awarded by lottery every three months.
Patients and caregivers who have received a Care for Life grant have told us how much it has meant to them. Some caregivers said the grant allowed them to hire a health-care worker for several hours each week and this gave them the opportunity to run errands, visit friends, or even just relax for a while. One patient said he used his grant so a health care worker could give him a proper bath and a shave each week. How you use it is up to you, so long as it is going to patient care and provides the caregiver with a much-needed respite. Care for Life is not intended to supplement existing care, but rather to assist families without any help. The grant is a one time award.
A licensed health care agency or professional must provide the respite service and bill Ride For Life, Inc. directly. They must also furnish us with their tax identification number. Our liaison will assist with agency inquiries about the respite grant.
There are four opportunities a year to apply for a grant with a new application period starting every 3 months. Non-selected patients are encouraged to reapply.
APPLICATION PROCEDURE
Please read the following information carefully before submitting an application.
Beginning September 10, 2011, Ride for Life, Inc. will be accepting applications from interested people. ALS patients, or their caregivers may apply for these grants.
All applications must be completely and correctly filled out and submitted by October 15, 2011.
During this phase of the program, a limited number of ALS patients will be selected from all applications in a random drawing to be held on November 1, 2011.
Please feel free to distribute this information and application form to other ALS organizations and support groups. We realize that not all ALS patients have access to the Internet and we would like to give as many families as possible the chance to participate in this program.
