An important part of Ride for Life's mission is to help ALS patients and their familes who are facing the daily challenge of living with the disease.
Through your generous contributions, Ride for Life has established a patient services program to address some of these challenges.
Ride For Life is pleased to announce next round of our very successful Care for Life program which provides grants for limited, in-home respite care for qualifying ALS patients.
Care for Life will greatly assist families by easing the strain, which many face when dealing with ALS because of the lack of respite care from their health insurance provider.
You will have 4 opportunities a year to apply for a grant.
A limited number of grants for up to $1500 each are available during this application period.
APPLICATION PROCEDURE
Please read the following information carefully before submitting an application.
Beginning March 1, 2007, Ride for Life, Inc. will be accepting applications from interested people. ALS patients, or their caregivers may apply for these grants.
All applications must be completely and correctly filled out and submitted by March 31, 2007.
During this phase of the program, a limited number of ALS patients will be selected from all applications in a random drawing to be held on April 4, 2008.
Each grant awarded will be in the amount of $1500.
Due to limited funding, Ride For Life, Inc. can only make one grant per ALS patient. You may enter for every (future) drawing until you are selected (so long as you continue to meet eligibility requirements).
Those who enter must stipulate that they are not currently receiving any reimbursed care, assistance, or are not eligible under the provisions of an insurance carrier or government program. This is to guarantee that only those in the greatest of need are considered for selection.
Care for Life is not intended to supplement existing care but rather, to assist families without any help.
Please Note: Funds must begin to be used within three months of receiving the award or you will forfit your grant.
If you are not in immediate need of respite care, please do not apply now. Apply in the future when you are in need.
A licensed health care agency must provide the respite service and must bill Ride For Life, Inc., directly. They must also furnish us with their tax identification number. Our liaison will assist with agency inquiries about the respite grant.
Selected families will be notified by either telephone, mail, or email, and are responsible for finding agencies and securing care.
Ride For Life, Inc. will pay the contracting agency directly a sum of up to $l,500 per patient for respite services provided.
One example of how this can be worked out is a respite care nurse assistant can come to your home for 4 hours a day, twice a week, for 6 weeks. Perhaps a skilled nurse can work several shifts providing the family a brief respite.
Should any dispute between patient and agency arise for any reason, Ride for Life, Inc. will be considered as a "Good Samaritan" and, as such, be held harmless. Selected participant will need to sign our corporate release.
Ride for Life, Inc. will have a liaison to assist families with program details.
Now that the Care for Life program is a proven success, Ride For Life, Inc. will continue to award new grants each quarter of the year.
Please feel free to distribute this information and application form to other ALS organizations and support groups. We realize that not all ALS patients have access to the Internet and we would like to give as many families as possible the chance to participate in this program.
